Common Irregularities Found in the Submitted Specified Forms (Form MBI 1, MBI 2, MBI 3, MBI 3a, MBI 4 and MBI 5)
- Address of building/premises does not match with the statutory notice number.
- Owner's/Owner representative's/Registered Inspector's name missing/signature outstanding.
- Incomplete/incorrect registration number of Registered Inspector.
- Signature of Registered Inspector different from this Department's records.
- Incorrect date of certification or declaration in MBI Form. (e.g. date of certification or declaration should be made on or after the date of inspection/completion.)
- Inspection/completion report not enclosed in submitted Form MBI 3, MBI 3a or MBI 4.
